Please consider this my formal application to join the Dwarf Athletic Association of Canada (DAAC). **Please note: This form is for DAAC information purposes only and does NOT register you for the World Dwarf Games (When available, there will be a separate application.) *Eligibility: You are eligible for competition as an athlete if you are a disproportionate dwarf that is less than or equal to five feet in height, or a proportionate dwarf less than or equal to 4’10” in height.

There are no membership fees to be part of DAAC. Dwarf Athletic Association of Canada is a Registered Canadian charity operated entirely by volunteers. Please consider a financial donation directly at: https://dwarfathletics.ca/donate or by sending an INTERAC email money transfer to: communications@dwarfathletics.ca Corporations may be able to deduct the donation as a marketing or advertising expense. DAAC Waiver of Liability, Release, and Indemnification and Hold Harmless Agreement *

DAAC requires all athletes to complete a Liability Waiver and a Media Waiver to participate in our programs and as a Team Canada member. Please complete the DAAC Liability Waiver and Indemnification Agreement form below. Dwarf Athletic Association of Canada Waiver of Liability, Release, and Indemnification and Hold Harmless Agreement – Section 1 In consideration of being allowed to participate in any way in the athletics/sports programs and related events and activities that the Dwarf Athletic Association of Canada (DAAC) may be associated, the application to join, volunteer, or participate as an athlete for DAAC agrees that DAAC, its directors, officers, servants, agents, successors or assigns, shall not be held liable for any injury, loss of damage, however caused, which the organization may incur resulting from or arising out of participating in any event for which DAAC may be involved. The applicant further agrees that it will from time to time, and at all times hereafter, truly save, keep harmless and fully indemnify DAAC, its directors, officers, servants, agents, successors and assigns from any and all actions, causes of actions, claims and demands whatsoever which may be brought against or made upon DAAC, its directors, officers, servants, agents, successors and assigns and against all loss, liability, judgments, costs or expenses which the DAAC its directors, officers, servants, agents, successors or assigns may sustain, incur or be put to resulting from or arising out of any act or omission on the part of the applicant, its servants, agents, successors or assigns which was done, or purported to have been done, in the performance of the applicant’s event/activity/participation obligations hereunder. Section 2 The undersigned hereby gives permission to DAAC to make available athletic opportunities for the applicant to participate in and to encourage participation in said activities. You acknowledge and fully understand that each applicant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, you accept and acknowledge that you have fully considered and understand the risks that the you could be physically injured as you prepare for, participate in and travel to and from the programs and Dwarf sporting events in which DAAC is involved. You assume these risks. In addition, you know of no reason why your participation in this program or its events should be exceptionally or unusually hazardous to your health. You assume ALL of the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death. Section 3 It’s important that the applicant know their own limits and abilities and play within them. Rules will be laid out prior to play and all participants are expected to abide by them. Proper equipment and clothing must be worn during all activities and is the responsibility of the applicant. By submitting this form you recognize the inherent risk that comes with playing any sport and will not hold DAAC or officials liable for any injury that may be sustained while participating or otherwise. Section 4 You agree that this release, waiver of claim, and assumption of risk is binding upon the applicant, their executors, their guardians, their administrators, personal representatives and assigns. Section 5 You understand that the collection of personal information about the applicant will be held in strict confidence and is to be used solely for the purpose of administering the athletic opportunities the applicant may be participating in. Section 6 You further agree the information about yourself or your child may be shared, at the discretion of DAAC, with athletic organizations and other partners involved so that the applicant’s needs may be best met. Section 7 You understand that this Waiver is the property of DAAC and for the purpose of my involvement with DAAC. You agree that you have read and understood this agreement.

Any photographs or video productions taken of the applicant during and at athletic activities may be used by DAAC for purposes of promotional material including brochures posters, newsletters, media information, advertisements, audio-visual productions and web pages, such as the Agency website and social media. Photographs or video productions may also be shared with community and Dwarf Athletic events planners and organizations for promotion.

I give consent that as a member of DAAC I authorize the organization to display, exhibit or otherwise communicate to the public my involvement for the purpose of publicizing DAAC and my involvement. * *
